In the All on Four/Six method, the implant site can be prepared and disinfected with a laser, and the implant and prosthetic teeth placed in the same day. This is same whether teeth need to be extracted, or there are no teeth to begin with.

In jaws that have been neglected for a long time due to various reasons, bone loss caused by gingivitis, combined with anatomical bone deficiencies (drooping sinuses in the upper jaw, superficial nerve bundles in the lower jaw, etc.) is difficult to produce bone with classical methods. Anatomical structures in the lower and upper jaws are different in people who have no teeth. Therefore, the technique applied and the number of implants to be applied may also be different.

In the upper jaw

The surface area covered by the teeth is wider than the lower jaw
The jawbone is of lower density than the lower jaw, therefore the integration of the implants with the bone is weaker and lasts longer
Especially the upper jaw anterior region is aesthetically important
If the sinus is drooping in the posterior region, the supporting bone is less, in this case, either angled implants should be placed, or short implants are used

When all these factors are brought together, as a result of the calculation made by the physician, mostly 6 implant systems (All on Six) are preferred in the upper jaw.

Mandible prostheses are usually satisfactorily retained by 4 implants, 2 anterior and 2 posterior. With correct planning and implant placement, the All On 4 technique is the most appropriate.
